    Colorado Accountable Care and Welltok Team Up to Guide and Motivate Seniors

    DENVER, COJune 19, 2014 – Today, Centura Health’s Colorado Accountable Care (CAC) announced it is leveraging the CaféWell Health Optimization Platform, developed by Welltok, to support the health and wellness of its senior population beyond the clinical setting. CAC joins a growing number of accountable care organizations (ACOs) creating innovative ways to partner with consumers to support healthy lifestyles with the goal of delivering better outcomes.

CaféWell will encourage social interactions among seniors and stimulate conversations through the use of condition-specific communities, such as for back and neck pain, hypertension or asthma. CAC users will have the opportunity to receive confidential, relevant support from health coaches and peers, and be eligible for rewards by taking steps towards improving their health.
