  • Annual Benefits Enrollment 2017-2018

    Centura Health’s benefit plan year runs from July 1, 2017 through June 30, 2018. If you would like to continue your medical and/or dental coverage, you must complete the Continuation of Coverage form and mail it to the Centura Benefits Service Center by June 17, 2017.

    For the upcoming year, we will continue to offer the UnitedHealthcare HRA Medical Plan and UnitedHealthcare HSA Medical Plan, both administered by UnitedHealthcare. In addition to these plans, Centura Health is also introducing a third medical plan called the Centura Value Plan, or CVP. The CVP option is currently only available to residents of Colorado, but we are exploring expanding coverage outside of Colorado for future plan years. 

    MetLife will continue to provide dental coverage for the 2017-2018 plan year. You may select from two dental plan options offered by MetLife. Regardless of the plan you choose, you pay no deductible for preventive services, and if you use a MetLife network dentist for preventive services there is no out-of-pocket cost to you and no dental claim to file for these services.

    Additionally, you may not add medical or dental coverage if you (or your eligible dependents, if applicable) have not been covered under the plan since your termination date. For example, if you are a medical COC participant, but you did not continue your dental plan, you may not add dental coverage at this time.

    Please complete the 2017-2018 Continuation of Coverage enrollment form if you would like to continue your coverage for the new plan year. The premiums will change for the Medical Plans effective July 1, 2017, so be sure to check the premium for the plan(s) you elect and make your monthly check for that amount. Your first month’s premium(s) at the new rate take effect July 1, 2017.

    **If you are currently on severance, the new premiums for the upcoming year will be deducted from any remaining severance payments that you are scheduled to receive following July 1, 2017.
