Physicians are the core of Colorado Health Neighborhoods. Our goal is to collaborate and join forces to build a sustainable health care system. Together, we’re working to raise the bar on quality to improve care for the communities we serve. 

Physicians are leading our efforts to create a health care model for the future through several key areas:

• Reduced overall health care costs and improved outcomes in defined populations. This will be accomplished through our focus on wellness and education to empower individuals to participate more actively in their own health care.
• Enhanced communication and care coordination across the system will result in less duplication and better planning, saving time and money while improving outcomes.
• Efficiencies, improved quality, access and service will result in increased reimbursement for participating physicians.
