We launched the Colorado Health Neighborhoods initiative to provide care that is vastly different from the fragmented health systems in most communities today, with robust care coordination that includes the support of coaches and navigators to help you manage the health care system. Colorado Health Neighborhoods will deliver high-value care at a local level.

Our new health neighborhoods approach includes acute care, the state’s largest multi-specialty physician practice, a health resource center, occupational health services, and home care and hospice, to make a wide array of health care services easily accessible. Each health neighborhood will be optimized to serve its community. In some areas, that might mean pairing a primary care practice with visiting specialists and telehealth services. In others, it will include bringing together, primary care, specialists, imaging and laboratory services, along with a complete acute care hospital.
