SET (Service, Empowerment, Transformation) Facilities – Centura Health

Centura Health is proud to sponsor three SET facilities located in Denver, Colorado Springs and Pueblo. These facilities strengthen Colorado communities by providing health care and other related services. Each led by a mission to provide quality care to the elderly, uninsured and underinsured, the following SET facilities embody the mission of Centura Health as they support our efforts to provide community benefits to Coloradans throughout the state.

HealthSET was created by the Wheaton Franciscan Sisters in 1988 who worked with different orders of the Sisters of Charity to replicate a successful model that had been developed in Milwaukee. The intent of the Sisters was to serve the poor, especially the frail and elderly population.

HealthSET is dedicated to improving the lives of the low-income elderly. Highly skilled and compassionate nurses and case workers provide free health clinics and access to community resources, acting as a bridge between low-income seniors and the complex health care system. By addressing the physical, emotional and spiritual needs of this vulnerable population, HealthSET helps seniors live independently for as long as possible while preserving their quality of life.

HealthSET’s mission is to extend the healing ministry of Christ by serving and empowering the low-income elderly in the Denver metro area through nursing, education and advocacy to transform the quality of their lives.

SET Medical Clinic at St. Francis Health Center provides basic health care to the uninsured, underinsured and low income people without discrimination and with the help of volunteer health care professionals.

Since its inception in 1990, SET has provided health and wellness services to the uninsured, low-income and homeless population of the Pikes Peak Region. By partnering with other agencies, SET provides unduplicated services to the community. SET receives no state or federal funding and subsists completely on grants and donations.

SET Family Medical Clinics’ mission is to provide basic medical services and holistic health programs to uninsured, underinsured and low income people without discrimination.

Pueblo StepUp is a 501 (c) (3) nonprofit organization in Pueblo that provides access to health care and wellness services for at-risk members of our community. Services provided by Pueblo StepUp include free senior wellness clinics; health promotion programs such as chronic disease self-management and fitness classes for the elderly; free application assistance for enrollment in the state’s low-income insurance plan (Child Health Plan Plus/CHP+) and Medicaid for children and pregnant women; and care management services for clients who are already enrolled in CHP+ and Children’s Medicaid. Pueblo StepUp is a Community Health Service Organization of Centura Health.
 

The mission of Pueblo StepUp is to positively impact the health, well-being and access to healthcare for Pueblo's underserved.
