Diagnostics

After a physician's physical examination, testing in a vascular laboratory is often the first step in diagnosing vascular disease. Specific tests that will be done will depend on the patient's symptoms and the suspected vascular problems. Diagnosing vascular disease begins by taking a careful medical history, including risk factors and a physical exam (including symptoms, temperature of the skin, appearance of limbs and blood vessels, and presence or absence of pulses). From this information, the physician decides if further testing is needed. 

Non-invasive testing utilizes various types of technology to evaluate flow, perfusion and pressures within the vessels at rest and with exercise. These procedures are generally painless and can help to determine if blood vessel disease is present, the location, and severity. From the results of these tests, the physician will determine the need for more non-invasive testing or procedures to treat vascular disease.

Non-invasive studies include:  

  • Carotid duplex
  • Abi's (ankle brachial indicies) with digit pressures
  • Upper and lower extremity arterial duplex
  • Upper and lower extremity venous duplex with reflux exam
  • PAP's/PVR 's (segmental and digital plethysmography and pulse volume recordings)
  • Bypass graft surveillance
  • Aortoiliac ultrasound

CommonSpirit Vascular Surgery Penrose offers comprehensive surgical services for patients in need of vascular surgeries. 

In this procedure, a device called an endoluminal stent graft (endograft) is placed inside the diseased artery to provide support for the weakened artery. The device allows blood to pass freely through the artery without pushing on the weakened walls of the aneurysm. This procedure can be performed in the thoracic aorta, abdominal aorta, iliac arteries, or popliteal arteries.

An IVC filter is an umbrella shaped device designed to catch life threatening blood clots from traveling to the lungs and causing pulmonary emboli (PE). IVC filters are used when a patient cannot safely take oral anticoagulants, when a patient has to suspend their oral anticoagulation, or when patients have blood clots that are refractory to anticoagulation. Some IVC filters are removable.

An aneurysm is a swelling or ballooning of an artery wall. This causes the artery wall to weaken and is life threatening if the artery were to burst. In open aneurysm surgery, an incision is used to open the skin, the artery is freed from the surrounding tissue above and below the aneurysm, and an artificial graft is inserted to replace the diseased artery. 

Many patients have clogging of the arteries that causes narrowing and eventual blockage of the arteries that supply blood flow to their legs. Our surgeons perform a variety of procedures to treat these blockages. Sometimes they can repair the artery from inside the blood vessel - using "endovascular" techniques similar to heart catheterization. Typical endovascular procedures include stenting and angioplasty. Other times surgeons have to perform open surgery to bypass and reroute around the blockage, using either a patient's vein harvested from another leg or an artificial artery.

A carotid endarterectomy is a procedure where a narrowed carotid artery is surgically opened and cleaned out to reduce the patient's risk of having a stroke. Our surgeons also perform removal of carotid body tumors, and perform endovascular carotid stenting in select patients.

Our surgeons create hemodialysis fistulas and grafts and place peritoneal dialysis catheters and for dialysis patients. They also perform maintenance procedures on these access sites when they develop problems.

Our providers evaluate and treat venous insufficiency and varicose veins. They start with an ultrasound evaluation to look for venous insufficiency - also called venous reflux – which is when the valves inside the veins do not close fully and allow blood to flow backwards towards the feet, causing swelling, pain, discoloration, and ulcers. Our providers also evaluate and treat varicosed – or enlarged and dilated – veins. Surgical treatment of these conditions include endovenous laser ablation, phlebectomy, and sclerotherapy.
