Porter Adventist Hospital and Senior Care of Colorado Announce Expanded Geriatric Clinic

(DENVER, COLO.) The Colorado Commission of Aging has identified Coloradoans as one of the fastest growing aging populations in the nation. The number of older adults in the state are expected to rise 54% from 2011-2021. To continue supporting this rapidly growing population, Porter Adventist Hospital and Senior Care of Colorado, a medical group practice of IPC Healthcare, Inc. are opening an expanded outpatient clinic conveniently located on Porters campus in August 2016.

The philosophy at IPC/Senior Care of Colorado strongly aligns with that of Porter Adventist Hospital, which is to provide a continuum of care for patients through a whole-person care approach. The medical team at IPC/Senior Care of Colorado sees patients in their outpatient clinics, surrounding independent and assisted living facilities, short-term rehabilitation facilities, nursing homes and inpatient hospital settings.

As a part of Centura Health, Porter Adventist Hospital has access to exceptional medical resources for geriatric patients. In collaboration with Colorado Health Neighborhoods and community partnerships, Centura Health is dedicated to providing quality senior care management through affordable care and access to physician practices, inpatient and ambulatory services. Centura Health also offers options in palliative care, hospice, telehealth services, assisted living, memory care, and rehabilitation therapies. In addition to services specific to the senior population, Porter Adventist Hospital has a strong network of specialists in cardiac, oncology, neuroscience, orthopedics and more.

Patients at IPC/Senior Care of Colorado admitted to Porter Adventist Hospital will have a dedicated team of IPC hospitalists and care coordinators providing high-quality patient care and an outstanding level of coordinated service. This team approach helps improve patients recovery and decreases the possibility of being readmitted to the hospital. The team maintains open lines of communication between patients and their families, the patients primary care physician, specialists and others involved in the patients care to ensure a smooth, well-planned transition between care settings.

Porter Adventist Hospital recognizes the need to address the unique health concerns associated with aging. We are thrilled to be working with IPC/Senior Care of Colorado in providing a spectrum of care tailored to the senior population, says Dave Dookeeram, Chief Operating Officer at Porter Adventist Hospital.

For more information about the expanded Senior Care clinic at Porter Adventist Hospital, a community open house will be held Friday, September 30 from noon 2:00 pm at the new clinic.
