Contractors

Centura Health has adopted a policy requiring all contract staff providing patient care and/or services within hospital patient care areas to comply with certain regulatory mandates. The purpose of this policy is to protect our patients and ensure that the requirements for on-site contract staff entering patient care areas are consistent with those required of employed staff.

These requirements include the following and must be made available on request:

  • Criminal Background Checks (consisting of a minimum of 3 years of history, and evidence that the contracted staff is not listed on the Office of Inspector General and Excluded Parties List systems)
  • Occupational Health Screening (consisting of TB every 2 years, MMR and chickenpox immunization screenings, and 7-panel + synthetics drug screen). Additional Occupational Health Screenings may be required of Direct Patient Care Providers.
  • Facility orientation, ID badging, and check-in / check-out, prior to performing work functions in a Centura Health hospital patient care area must be completed.

Orientation Materials

Select a contract staff category which applies to you for the appropriate orientation materials.

  • Category 1 - Clinical Direct Patient Care

    Contract Staff assigned to patient care where contact with patients may range from minimal to full care. These Contract Staff have access to personally identifiable health information; report to nursing or other clinical department for assigned care, treatment or service.

    Facility Training material, and H-Series forms, and badging instructions are available below or at your local Centura Health facility's Human Resources Department or Professional Staffing Office. All requirements are to be met or contract staff will not be allowed to work within the facility.

    The H-Series forms must be completed by the contracting company for each contracted staff member, attesting that their authorized personnel have met all the requirements as indicated on the forms. Contracted staff will bring their signed H-Series forms, along with required documentation to their local Centura Health facility's Professional Staffing Office. Clinical Direct Patient Care Contract staff will continue to sign in and out daily at the designated department and/or staffing office.

  • Category 2 - Outsourced Services

    Contract Staff assigned to services that would normally be a function of a Centura Health Hospital, but the Hospital has chosen to provide that function through an external company. These Contract Staff may serve in functions that include, but are not limited to, services such as nutrition services, environmental services and/or physical medicine.

  • Category 3 - Clinical Indirect Patient Care

    Contract Staff not assigned to patient care, though have minimal patient care contact; may perform interviews or limited assessments for screening, discharge coordination, or measurements. These Contract Staff may have access to personally identifiable health information; and are not consistently escorted in patient care areas.

    Facility Training material, and Attestation Form are available below or at your local Centura Health facility's Human Resources Department. All requirements are to be met or contract staff will not be allowed within the facility.

    The Non-Facility Personnel Attestation Form is to be completed by the contracting company for each contracted staff member, attesting that their authorized representative has met all the requirements as cited in this letter. Contracted staff will bring their company-signed Attestation Forms to your local Centura Health facility's Human Resources Department, where contract personnel will receive their new facility-issued picture badge with annual expiration date. Contract staff will continue to sign in and out daily at their designated department.

  • Category 4 - Construction/Facilities Contractors

    Contract Staff providing non-patient care services, including construction, facility-maintenance, and transportation services. These Contract Staff may interact with staff regarding patient care, but do not interact with patients unless supervised.

    Facility Training material, and Attestation Form are available below or at your local Centura Health facility's Human Resources Department. All requirements are to be met or contract staff will not be allowed within the facility.

  • Category 4 - Service Representatives

    Contract Staff providing non-patient care services, including vendor, supplier, delivery, and disposal. These Contract Staff may interact with staff regarding patient care, but do not interact with patients unless supervised.

    Centura Health departments and associates are not obligated or required to meet with vendors. All Vendors are required to be members of our current Vendor Management program and comply with the requirements determined by Centura Health Supply Chain Management. Vendors are seen by appointment only. Any out of town Vendor Representatives not registered in Centura Health Vendor Management program may be granted a visitor's pass but must accompany a Vendor Representative already registered. The visiting Vendor Representative will be expected to comply with all regulations in place and it will be the responsibility of registered Vendor Representative to ensure compliance standards are met.

    If a Vendor Representative is found visiting departments within the facility without first checking in at the facility's designated Supply Chain Management location and Vendor Management program to obtain a badge, this will be reported to on-site Supply Chain Management and may lead to a Security escort from the premises and further Restricted or Revoked privileges. Failure to comply with this policy will result in a request to the respective company by Centura Health Supply Chain Management for the vendor representative to no longer represent product/equipment/services at any Centura Health facility. In the event that Centura Health Vendor Management program system is unavailable due to any unforeseen circumstances, all Vendor Representatives will be required to check in at the facility SCM office to obtain a badge. Displays are not allowed in any facility without the expressed consent of the Corporate Vice President of Supply Chain Management or the Site Director of Supply Chain Management.

Penrose Hospital or St. Francis Medical Center Contractor Information

  • Contractor Information

    If you are working at Penrose Hospital or St. Francis Medical Center in Indirect Patient Care or in a Non-Clinical Role (Construction/Facilities), proceed to the Penrose St. Francis Indirect Clinical, Service Representative, Construction/Facilities below.

    Examples of Indirect Clinical contract labor include:

    • Insurance liaisons
    • Hospice/ home care workers, and sign language interpreters

    Examples of Service Representatives and Construction/Facilities personnel include:

    • Paper shredder disposal
    • Couriers
    • Construction labor
    • Hazardous waste removal
    • Facility maintenance

    Clinical In-direct, Service Representatives, and Construction/Facilities Contract Personnel

    Review the following:

    Once you have reviewed the information above, complete the following:

    1. Print out, complete, and sign the Centura Health Non-Facility Personnel Attestation Form
    2. The completed Centura Health Non-Facility Personnel Attestation Form must be returned to the Penrose-St. Francis Human Resources Department. Processing time for Non-Facility Personnel Attestation Forms is 24 business hours. After that time the contractor can go to the badging office for their badge.
    3. A contractor badge with an expiration date will be provided. You must wear your contractor badge on your upper right chest at all times while working in the facility.
    4. To renew a badge, review the Centura Health Non-Facility Personnel Initial and Annual Orientation Training, complete a new Non-Facility Attestation Form and return it to the Penrose St. Francis Human Resources Department. After 24 business hours of turning in the new form, go to the badging office with your current badge. The badging office will renew your badge for another year.
