Medical staff sponsored rotations
For medical students and residents, as well as mid-level provider students to include physician assistant students, advance practice nurse students (such as nurse practitioner and nurse midwife) or others who will rotate with physician or PA members of the medical staff at Penrose Hospital and St. Francis Hospital.
An affiliation agreement between your academic institution and Centura Health must be in place prior to accepting any rotation application. For clarification, it is the responsibility of the student and the academic institution to set up the rotation with a physician. We do not provide placement. All documentation indicated below must be received by the appropriate Centura Health Medical Staff Services Department at least 15 days prior to the start of the requested rotation date. Failure to provide this information 15 days in advance shall result in postponement or delay in beginning the rotation. Review the medical staff rotations and observations.

General student rotations
For all other students, clinical and non-clinical, not included in the categories above. Included are students in cardiology, imaging, nursing (including return to practice), nutrition services, pharmacy, physical therapy, occupational therapy, speech language pathology, respiratory therapy, social worker, surgical technician, exercise physiology, HIM, administration, marketing, finance.

A completed Attestation Form Letter must be received at least 15 days prior to the start of the requested rotation date. Failure to provide this information 15 days in advance shall result in postponement or delay in beginning the rotation.

Complete the following:
- School responsibility
- Email completed Attestation Form Letter and IT Request form for Student Computer Access if required.
- Student responsibility
- Read the Student Orientation Packet and Centura Health Integrity Standards
- Print, complete and return the Student Orientation Post Assessment to Penrose Hospital's or St. Francis Hospital's Education Resources Department.
