Our Services
Bristlecone Health Services will work with you and your physician to come up with a plan of care that meets your needs and helps you heal. In addition to providing excellent clinical care, Bristlecone Health Services knows that faith and inner strength can help with healing. That's why we also give special attention to your spiritual well-being.
Home care services
Bristlecone Health Services offers an interdisciplinary team of clinicians for our home care patients, including:
Skilled Nursing Care
Our registered nurses provide skilled care such as end of life care, wound care, medication monitoring, disease and nutrition education, and administration of I.V. medications.
Therapy
Our physical and occupational therapists provide rehabilitative exercise programs and teach appropriate use of medical equipment and safety measures, in addition to ensuring the home environment doesn't pose any hazards. Our therapists will help you regain your strength and range of motion so that you can return to your normal daily activities.
Personal Care Services
Certified Nursing Aides can provide intermittent assistance with personal care needs such as bathing and meal preparation.
Social Work
Medical social workers are available to connect you with community resources for additional help.
Spiritual Care
Non-denominational chaplains can visit with you to provide support and education to enhance your spirituality as you strengthen your body and soul.
Hospice care
Home hospice care is designed for those who wish to remain in their homes at the end of life.
Members of the hospice team will visit your home to make sure you are comfortable and well cared for. You and your family will help plan the kind of care you receive and will play a vital role in communicating your needs, concerns or questions to the members of the hospice team. To provide the best quality of care, we provide our hospice patients with a comprehensive team to best meet their needs. Hospice team members include:
Your Physician
Your primary physician will continue to provide medical care and will work with the hospice team in providing your comfort care.
Medical Director
The hospice Medical Director works with your physician as an additional resource for your comfort. The Medical Director oversees the ongoing review of your need for hospice care. If necessary, the Medical Director may offer consultation to ensure you receive appropriate treatment that supports your goals.
Registered Nurse
The hospice nurse will work closely with your physician ensuring you are as physically comfortable as possible. Helping with pain and other symptoms are our primary concerns. Your hospice nurse will make scheduled visits. Nursing staff are on call 24 hours a day, 7 days a week for phone consultations or emergency visits.
Social Worker
Clinical social workers provide support for you and your family by offering emotional comfort, assistance with community referrals and short term counseling. Emotional support is available 24 hours a day 7 days a week to address any concerns that may arise.
Chaplain
Spiritual issues are often important at the end of life. The non-denominational hospice chaplain assists you in addressing these issues. Spiritual and religious beliefs are as individual as people. The chaplain assists you and your loved ones by using your own faith or beliefs as a means of coping with your illness.
Personal Care Aide
Certified nursing aides assist with personal care such as bathing and other hygiene needs. The health care aide can also help with light household duties related to your care.
Hospice Volunteer
Every hospice volunteer is required to complete comprehensive training. Volunteers assist in a variety of ways to give family members time to rest or maintain contact with friends. Volunteers can assist with light housework, running errands and providing companionship for the patient and caregiver.
Other Team Members
Other support team members may include a physical therapist, occupational therapist, dietitian and bereavement counselor.
Bereavement & grief counseling
Bristlecone Health Services understands that grief often brings up unfamiliar and sometimes confusing emotions and responses. That's why we offer grief and bereavement counseling to support the needs of both family members and friends that have experienced the death of a loved one. Individual grief and bereavement counseling is available to all community members, including our hospice families free of charge.
Volunteer
Some of the most valuable contributions come not from the wallet, but rather from the heart. Our programs were truly founded in this spirit of caring and volunteerism.
At Bristlecone Health Services, we value and respect your time and willingness to volunteer-our programs couldn't operate without you.
We are currently seeking volunteers for the following assignments:
- Be a friendly visitor by providing companionship to patients in their own homes or nursing homes. Visits may include reading, writing letters, running errands or preparing lunch. A friendly visitor is there to meet the patient's need and energy level.
- Provide caregiver relief by staying with and visiting the patient at home while the primary caregiver gets a much-needed reprieve.
- Bereavement volunteers follow up with family members who have lost a loved one in our program, providing visitation and caring support.
- Administrative assistance usually involves mailings, filing, photocopying, assembling packets, couriering, or other office related assistance.
- Serve on a fundraising committee for events like Tree of Memories or The Legacy Forest.
