There are certain services, chronic conditions or on-going care for which Centura Health may not be able to provide customized pricing without additional clinical information from your physician.
Effective January 1, 2021, CMS requires hospitals to make available a list of their current standard charges, cash prices of services for self-pay and uninsured patients, insured patient out-of-pocket pricing on a select group of common services, payor specific contract rates for services and the de-identified minimum and maximum contract rate for each service. If a patient has health insurance, the patient’s health insurance policy (including deductibles, co-pay, co-insurance, and out-of-pocket maximums) will apply and the amount the patient owes for health care services will depend on the patient’s insurance coverage. If you are covered by health insurance, you are strongly encouraged to consult with your health insurer to determine accurate information about your financial responsibility for a particular health care services provided by Centura Health. If a patient chooses not to utilize their insurance coverage and instead wishes to be considered a self-pay patient, the patient may request to have their insurance information removed from their patient record and must sign a waiver of insurance coverage (except Governmental payers).
